WMYSL 2009 Registration Information
For Boys and Girls 6 to 19

THE REGISTRATION DEADLINE IS FIRM. ALL PLAYERS WHOSE FORMS ARE SENT IN AFTER JUNE 30, 2008, WILL BE PLACED ON A WAITING LIST AND WILL BE PLACED ON A TEAM ONLY AS SPACE BECOMES AVAILABLE.

Fees:
$100 Registration fee for the first player in the family, if the fee is mailed on or before, June 30, 2007
$25 Additional late fee for the first player in the family, if the fee is mailed after June 30, 2008
$60 Siblings registration fee for each additional player in the family, regardless of when the fee is mailed
$50 NOT Volunteer fee
$60 U6 Academy

Submission Requirements
1. Payment must accompany the registration form, or the form will be returned.
2. The registration form must be signed in both places by a parent.
3. Checks are to be made payable to: “WMYSL”.
4. For ALL U12, U14 & U16 players, a copy of the player’s BIRTH CERTIFICATE or PASSPORT and a 1” x 1” head shot picture must accompany the registration form and the check.

WMYSL Recreation Volunteer Program
Our recreational soccer program is composed entirely of volunteers, so we always need parental assistance. We request that you sign up to volunteer in one of the vollunteer positions listed on the registration form or make a $50 contribution by adding that amount toyour registration check.

TEAM SPONSORS have also assisted in keeping down our registration fees. Sponsorships are $300. If you would like to sponsor a team, please see the Sponsors page and so indicate on the registration form. SOCCER UNIFORM JERSEYS WILL BE COLLECTED AT THE END OF THE SEASON TO HELP SAVE THE LEAGUE COSTS ON UNIFORMS. Contact Dominique Sperry at 457-5878.
